APPROVAL FORM. NO EXCEPTIONS!! PLEASE BE SURE TO LOOK OVER ARTWORK
CAREFULLY CHECKING SPELLING, PUNCTUATION, AND LAYOUT. |

INK CHANGES: When the color of imprint is changed on the same order, $15.00
minimum.
SPECIALTY INKS: Puff, Metallic, neon etc. $0.25 per color to running charge.
SCREEN CHARGES: $25.00 per screen (a new screen is required for each different
color used)
REORDERS: If you are placing a re-order, you will not pay an art charge, unless
we are editing your art. However, screen charges still apply on re-orders,
because we are unable to keep screens after the completion of the order. |
